If you’re a brain injury or stroke survivor, a family member, a caregiver, or a community healthcare provider outside Alberta Health Services Calgary Brain Injury Program, you can initiate the application process using the forms provided below. Please review the eligibility and exclusion criteria for survivors before applying.

Financial Assistance

Eligibility & Exclusion Criteria for Survivors

Exclusion Criteria

• Rehabilitation Services: Have a moderate to severe impairment from an acquired brain injury within the past 5 years.
• Wellness Services: Have a moderate to severe impairment from an acquired brain injury.
• Recreation Programs: Open to all levels of acquired brain injury.
• Medical documents supporting the diagnosis of a brain injury will be requested.
• 18 years of age or older
• Determined medically stable by a physician
• Resides within the boundaries of the Calgary Region
• Has functional or recreational goals related to participation in the context of their residence or community
• Has the endurance required to participate in a therapeutic program

• Hospitalized as an inpatient, including transition units
• Has congenital/development problems not considered to be an acquired brain injury (e.g. cerebral palsy, developmental delay, down syndrome)
• Has a progressive disorder/disease not considered to be an acquired brain injury (e.g. Alzheimer’s disease, dementia, multiple sclerosis, Parkinson’s disease)
• Active substance abuse or misuse
• Client or support person(s) behaves in a manner inconducive to a therapeutic environment
